Multi Factor Authentication Our website and mobile application require Multi Factor Authentication. This involves the use of at least two different levels of authentication to gain access to the secure areas of the website and mobile application. You can choose the method of authentication during registration. Electronic Documents Delivery and Right to Request Paper / How to Withdraw Consent Users of the website and mobile application may have the ability to opt to receive Explanation of Benefits statements (EOBs) as well as other plan information and disclosures electronically. On nwadmin.com: 1. Log on and go to Edit Account. 2. Go to Change EOB and other Document Delivery Preferences. 3. To opt to receive electronic EOBs, select "I want to go GREEN and get electronic EOBs." 4. To opt to receive other plan related communications electronically, select “I want to go GREEN and receive other plan communications electronically.” On the NWA mobile application: 1. Log on and go to Account Preferences. 2. Toggle Electronic EOBs to receive your Explanation of Benefits electronically. 3. Toggle Other Documents to receive other documents electronically, as they become available. To resume paper delivery of EOBs, uncheck the "I want to go GREEN and get electronic EOBs” option or toggle ‘off’ for Electronic EOBs. To resume paper delivery of other plan communications, deselect “I want to go GREEN and receive other plan communications electronically” or toggle ‘off’ for Other Documents. Electronic Delivery Method to Be Used: Documents will be viewable within your online account; you will receive an email when a new EOB or other communications are posted. Documents will be available using Adobe Acrobat Reader. To access the documents, you must have (1) a device with internet access; (2) a program installed on the device that allows you to send and receive e-mails; and (3) Adobe Acrobat Reader installed allowing you to open and read the posted document. To retain a copy of the e-mail and for future reference, you must either (1) be able to print a copy on a printer; or (2) save a copy in electronic form. If any of these requirements change in a way that creates a material risk that you will no longer be able to access and retain electronically transmitted documents, you will be furnished with notice and required to provide consent for receiving documents electronically. Your Right to a Paper Copy: You have a right to request and obtain a paper version of any electronically transmitted document at no charge. Contact the administrative office to request a paper copy.
